ABSTRACT

Introduction: Continuous procurement of various equipments in the hospitals is essential to keep up with global progress of healthcare services. In public sectors, procurement process largely depends on budget sanctions and also by a number of formal procedures and rules which leads to delayed procurement. With this prevalent scenario, the present study was carried out to evaluate the procurement of two major equipments in an apex tertiary care institute using Project Evaluation and Review Technique (PERT). Objective: This study was done to evaluate the procurement of two major equipments using PERT in a tertiary care institute. Method: This study was conducted using Operational Research technique in an apex tertiary care institute i.e., medical college and hospital, in one of the North Eastern states of India. All the procurements related to equipments in that teaching hospital initiated in the financial year 2021 were listed. Out of them, two procurement files were selected by simple random sampling; procurement of Operating Table and Bio Safety cabinet Level – III. Project Evaluation and Review Technique (PERT) was applied to calculate the possible reduction in time in the whole procurement process. Results: The present study showed that procurement of Operating table took 185 days where minimum 8 days could have been saved and procurement of Bio-Safety cabinet Level-III was completed by 702 days where minimum 196 days could have been saved. Conclusion: Application of PERT in this procurement evaluation process can play significant role by calculating critical path and predicting the possible delay.

ABSTRACT

Objective:To investigate and analyze the allocation and use of the common pediatric medical equipment and drugs in community health service centers, so as to provide decision-making reference for further strengthening the capacity-building of community children′s health services.Methods:Using multi-stage stratified random sampling method, 30 community health service centers in 14 cities were selected to investigate the basic situation of the sample centers, and the allocation and use of 21 kinds of commonly used pediatric medical equipment and 23 kinds of commonly used children′s drugs from April to August 2020.Results:The overall allocation rate of 21 commonly used pediatric medical equipment in the sample centers was 61.9%, and the overall utilization rate was 62.6%. The overall allocation rate of 23 commonly used drugs for children in the sample centers was 46.67%, and the overall utilization rate was 31.1%. There was a statistically significant difference in the utilization rate of equipment among different levels of urban community health service centers ( P<0.05); There were statistically significant differences in the allocation rate of commonly used children′s drugs among community health service centers under different regions, cities at different levels, types of jurisdictions, and institutional types ( P<0.05). Conclusions:The overall allocation and utilization rate of commonly used pediatirc medical equipment and drugs in urban community health service centers in China were relatively low, especially the drug utilization rate. The author suggested that the government should further strengthen the construction of grassroots pediatric diagnosis and treatment service capabilities, and reasonably equip commonly used pediatric medical equipment and drugs at the grassroots level.

ABSTRACT

Objective To comprehensively analyze and understand the status of medical resources of radiological diagnosis and treatment in Suzhou, China, and to provide a basis for the health administration departments to reasonably plan and allocate medical radiation resources. Methods The radiological diagnosis and treatment institutions were registered on the radiation health information platform of Jiangsu province, and information was entered as required. Results There were totally 793 radiological diagnosis and treatment institutions at all levels in Suzhou, including 22 (6.04%) tertiary institutions. There were 2208 radiological diagnosis and treatment equipment. The number of X-ray diagnosis and treatment equipment per million people in Suzhou was 205.40. However, there was no class A large-scale medical equipment. Conclusion Compared with 2005, the numbers of radiological diagnosis and treatment institutions and equipment in Suzhou increased significantly. However, government departments at all levels should strengthen overall regulation and control to improve the rational allocation of high-tech medical resources.

ABSTRACT

Objective:To explore the module of the construction and application of medical first-aid at the door of the space capsule and the air evacuation.Methods:According to the purpose, principle, and method set by the module, it was divided into two sub-modules: medical first-aid at the door of the space capsule and the air evacuation. During the comprehensive first-aid training, independent training and combined training were carried out respectively according to different mission stages of launch and recovery and different recovery terrain. Meanwhile, research was performed to ensure that medical carrying equipment was lightweight, modular, and portable, and the efficiency of modularization construction was tested in the comprehensive training.Results:The module of medical first-aid at the door of the space capsule and the air evacuation obviously shortened the rescuing time during the comprehensive training, the saving effect was remarkable, and the overall saving efficiency was significantly improved.Conclusions:The module of medical first-aid at the door of the space capsule and the air evacuation meet the requirements that the emergency support system of aerospace medicine should function on an immediate basis, ensuring accurate treatment and air evacuation without any delay, so that to achieve the aim of aerospace medicare.

As the medical equipment management department, the medical devices department is mainly responsible for the procurement, supply, technical support, management and coordination of medical equipment and medical consumables, playing an important role in epidemic prevention and control. Through the analysis of the expansion cases of designated hospitals, the experience of emergency management of medical equipment has been accumulated, which has strong practicability and replicability.

ABSTRACT

OBJECTIVE@#In order to record the whole life process information of medical equipment and improve the management efficiency, this study proposes an informatization scheme for the whole life cycle management of medical equipment.@*METHODS@#Relying on cloud-native, based on the software architecture of front-end and back-end separation, a mode of sub-account and sub-authority management of medical equipment was designed to integrate maintenance work orders, operation reports and other functions, and manage medical equipment data in a structured and platform-based manner.@*RESULTS@#Comparing the equipment failure rate, maintenance response time, and average inventory time per device before and after the system operation, the differences are statistically significant (P<0.05).@*CONCLUSIONS@#The system can realize the work collaboration of maintenance engineers, equipment users, and equipment management personnel, and can trace the information of the whole life cycle of the equipment.

ABSTRACT

El presente estudio de factibilidad técnica realizado por docentes investigadores del Centro Regional MEGATEC Zacatecoluca se centró en desarrollar una investigación descriptiva y aplicada para escalar la comunicación de un Sistema Central de Monitoreo de Pacientes a dispositivos móviles. El objetivo principal del proyecto fue brindar alternativas para escalar y optimizar el acceso a la información de los signos vitales de pacientes, a través de la integración y desarrollo de herramientas informáticas y aplicaciones de terceros, con el software del sistema y componentes informáticos del Sistema Central de Monitoreo. El proyecto integró herramientas, conocimientos, habilidades, experiencias, métodos y procedimientos requeridos para el desarrollo e implementación de un sistema de comunicación interoperable entre aplicaciones heterogéneas. Se utilizaron protocolos y estándares para intercambiar información entre aplicaciones de salud HL7 y FHIR.


This technical feasibility study carried out by research professors from the Centro Regional MEGATEC Zacatecoluca focused on the developing of a descriptive and applied research to scale the communication of a Central Patient Monitoring System to mobile devices. The main objective of the project was to provide alternatives to scale and optimize access to information on the vital signs of patients, through the integration and development of computer tools, and third-party applications with the system software and computer components of the Central System Monitoring. The project integrated tools, knowledge, skills, experiences, methods and procedures required for the development and implementation of an interoperable communication system between heterogeneous applications. Protocols and standards were used to exchange information between HL7 and FHIR health applications.

ABSTRACT

Introdução: A pandemia causada pelo coronavírus SARS-CoV-2 trouxe uma pressão descomunal sobre os sistemas de saúde, especialmente sobre a disponibilidade de leitos, equipamentos e recursos humanos das unidades de terapia intensiva (UTI), que mesmo antes desse cenário já apresentavam dificuldades, em especial na gestão de equipamentos. Embora se tenha passado mais de 10 anos do início da obrigatoriedade da gestão de tecnologias em serviços de saúde, a sua implementação na prática ainda é um desafio e um problema de saúde pública. Objetivo: Verificar a implantação do Plano de Gerenciamento de Tecnologias de equipamentos médico-assistenciais nas UTI de Goiânia, parte integrante da gestão de tecnologias. Método: Como base utilizou-se dados secundários coletados em um guia elaborado pela Vigilância Sanitária, aplicado em dois momentos durante as inspeções em UTI. Os dados foram analisados de forma comparativa e os resultados apresentados por meio de frequência absoluta, relativa e de análise estatística. Resultados: Os níveis de implantação do Plano de Gerenciamento de Tecnologias encontrados foram de 25,8% e 40,9% na 1ª e 2ª inspeção, respectivamente. Conclusões: Investimentos em treinamento e em programa de educação permanente podem levar a uma melhoria na implantação do plano e, consequentemente, a um avanço na qualidade do serviço oferecido ao usuário. Considerando que a Vigilância Sanitária é um importante catalisador dessa mudança, este estudo traz dados importantes para os gestores priorizarem ações e formularem políticas públicas na Saúde Coletiva que servirão para melhorar a segurança dos pacientes e, por consequência, ajudar no enfrentamento da COVID-19.


Introduction: The pandemic caused by the SARS-CoV-2 coronavirus has brought a huge pressure on health systems, in particular the availability of beds, equipment and human resources in Intensive Care Units (ICU), which even before this scenario already had difficulties, especially in equipment management. Although more than 10 years have passed since the beginning of mandatory technology management in health services, its implementation in practice is still a challenge and a public health problem. Objective: To verify the implementation of the Medical Equipment Technology Management Plan in the ICUs of Goiânia, Goiás, an integral part of technology management. Method: As a basis, secondary data collected in a Guide prepared by the Sanitary Surveillance was used, applied in two moments during inspections in the ICU. The data were analyzed comparatively and the results presented through absolute and relative frequency and statistical analysis. Results: The levels of implementation of the Technology Management Plan found were 25.8% and 40.9% in the 1st and 2nd inspection, respectively. Conclusions: Investments in training and in a permanent education program can be a direction to improve the implementation of the plan and, consequently, an advance in the quality of the service offered to the user. Considering that Health Surveillance is an important catalyst for this change, this study provides important data for managers to prioritize actions and formulate public policies in Public Health that will serve to improve patient safety and, consequently, help in coping with COVID-19.

ABSTRACT

Maintenance and quality control management of large medical equipment is 3-level qualitative index for performance appraisal of tertiary public hospitals, which affects their medical quality directly. Since 2015, Shanghai Medical Equipment and Device Management Quality Control Center and other government organizations have gradually established and improved the quality evaluation index system of large medical equipment maintenance in combination with China′s performance testing standards for large medical equipments and the technical requirements of various manufacturers, and putted the system into application. In 2018, the Quality Control Center further optimized the quality evaluation index system of large medical equipment maintenance by improving the maintenance sheet and service report of large medical equipments, using the " internet+ mobile terminal" to dynamically collect core data of equipment′s full cycle maintenance. This practice has established a regional database of large-scale medical equipment maintenance, building a comprehensive quantitative evaluation indexes. The maintenance quality evaluation scores of mainstream manufacturers of large medical equipments in Shanghai maintained an overall rising trend year by year, and the annual inspection compliance rate of CT and MRI maintenance quality has continued to increase from 2015 to 2021. This evaluation system enables the hospital management departments to supervise the manufacturers to complete their maintenance work at a high quality, ensure the safe operation of large medical equipments, and provide data reference and technical support for the qualitative evaluation of the maintenance quality of medium and large medical equipments in the " national examination" .

OBJECTIVE@#In order to grasp the quality status of the first-aid ventilator in pre-hospital and field environment in time, the quality control and detection items of invasive ventilator were optimized to form a rapid detection operation process of first-aid ventilator and ensure the safety of patient treatment.@*METHODS@#Based on the quality control detection data of invasive ventilator in hospital in recent years, methods such as narrowing the range of qualified criteria and analyzing confidence interval were adopted to extract indicators prone to deviation and verify their correlation with similar indicators, so as to form an optimized rapid detection portfolio.@*RESULTS@#Compared with the original quality control detection procedures, the detection indicators of the rapid detection procedures were reduced from 5 categories of 24 indicators to 3 categories of 7 indicators. The detection time was shortened by 56.1% and the coincidence rate of the detection results was 100% after the actual measurement and verification.@*CONCLUSIONS@#Under the premise of ensuring the testing quality, the operating procedure for rapid detection of emergency ventilator can greatly reduce the detection time, and realize the rapid and high frequency quality control detection, so as to ensure the quality and safety of the equipment.

With the deepening of China's medical reform, the scale of hospital equipment assets at all levels is also expanding. In the face of large-scale and various equipment assets, the traditional single machine statistical management method not only has a single interface, poor data accuracy and is not compatible with other systems, but also is difficult to achieve "full life cycle" management. In order to solve above problems, the Ninth People's Hospital Affiliated to Shanghai Jiao Tong University of Medicine aims to use the Internet of Things to build a medical equipment management information platform, realize the whole hospital area, whole subject and whole process management of medical materials, and achieve the objectives of business linkage, information connectivity and data sharing between management departments and clinical departments.

It is urgent to promote the China-led international standards of acupuncture and moxibustion and the patent protection of traditional acupuncture and moxibustion. Intelligent medical equipment will play the role of media, and promote the internationalization of traditional acupuncture and moxibustion with standardized and highly-integrated intelligent medical equipment for acupuncture and moxibustion, and integrate modern scientific and technological achievements. It is a feasible way for the internationalization of acupuncture and moxibustion.

Aiming at the current situation of medical equipment management, Shanghai Sixth People′s Hospital Affiliated to Shanghai Jiao Tong University School of Medicine explored the application of 5G+ Internet of Things and digital twin technology in the intelligent management of medical equipment. Through the intelligent perception terminal, the hospital realized the dynamic acquisition of real data such as the use location, use process and use efficiency of medical equipment, and conducted digital, networked and intelligent monitoring and analysis. At the same time, applications such as medical equipment dynamic management software, medical equipment real-time positioning software, and space environment quality monitoring software were developed. It realized automatic, intelligent and visual management of medical equipment configuration, operation and performance evaluation, achieved fine management based on digital twins, and improved the efficiency of collaborative management of operational resource support.

El mantenimiento preventivo en las instituciones hospitalarias se ejecuta para minimizar las acciones correctivas. Esto trae consigo una disminución de los gastos, una reducción de los riesgos y un aumento de la disponibilidad técnica de los equipos médicos, con la disminución de los tiempos de parada de los servicios, lo cual redunda en la excelencia de los servicios médicos prestados. En el campo de las tecnologías médicas ocurre un gran avance cada día, por lo que los equipos son más sofisticados y costosos. Este artículo tiene el objetivo de exponer la importancia de la capacitación ineludible del personal de Electromedicina en la actividad de gestión de mantenimiento a equipos médicos, a partir de las necesidades que ofrece la interpretación de su inventario. Se brindan pautas básicas sobre la preparación permanente y continuada del electromédico en mantenimiento a equipos médicos y se demuestra lo útil que resulta un inventario para gestionar la capacitación de los recursos humanos, lo cual se debe a la consulta, el análisis-síntesis y la sistematización de múltiples fuentes bibliográficas impresas e insertadas en la web: bases de datos Medline, Scielo, Redalyc y Google Académico. De ellas se revisó el material de 62 y, finalmente, se seleccionaron 24 artículos. Se concluyó que la capacitación en esta temática es fundamental para que las tareas de mantenimiento constituyan en verdad herramientas eficaces, que beneficien no solo a la institución prestadora de servicios de salud, sino al paciente y al personal médico(AU)


Preventive maintenance in hospital institutions is carried out to minimize corrective actions. This brings about a decrease in expenses, a reduction of risks and an increase in the technical availability of medical equipment, with a reduction in the times that services stop, which results in the excellence of the medical services provided. In the field of medical technologies, great advances occur every day, the reason why pieces of equipment are more sophisticated and expensive. This article aims to expose the importance of the unavoidable training of electromedicine personnel in the activity of maintenance management for medical equipment, based on the needs offered by the interpretation of its stock. Basic guidelines are provided about the permanent and continuous training of the electromedicine technologist in maintenance of medical equipment, while it is demonstrated how useful an equipment stock is to manage the training of human resources, which responds to the work developed after consultation, analysis-synthesis and systematization from multiple bibliographic sources printed and inserted on the databases of Medline, Scielo, Redalyc and Google Scholar. Of these sources, the material of 62 was reviewed and, finally, 24 articles were selected. It was concluded that training on this subject is essential for maintenance tasks, which truly constitute effective tools, benefiting not only the institution that provides health services, but also the patient and the medical personnel(AU)

Based on ASP.NET framework, The Intelligent Estimated System for Rational Deployment of Medical Equipment (MERDIS) is designed and developed with SQL Server 2012 database and C# language. The system is used to realize the rational deployment suggestions and evaluation of medical equipment in hospitals. The system input the data of hospital medical equipment and clinical pathway into the database, and then feedback the deployment information to users which are calculated by big data information, so as to achieve the purpose of giving rational deployment of hospital medical equipment.

ABSTRACT

In view of the inherent drawbacks of traditional medical equipment procurement mode, a decision-making model of medical equipment procurement based on Improved Markov model is proposed and the engineering evaluation practice is carried out. The data pool of medical equipment procurement of big data level is constructed, the core constraint factors of medical equipment procurement are perceived by deep learning, the decision model of medical equipment procurement under multi-dimensional constraints is constructed, and the Improved Markov model is introduced, and the observable decision-making scheme of medical equipment procurement is given. The results show that the model can give the decision-making scheme of medical equipment procurement with global optimal attribute under multi-dimensional constraints. It has obvious advantages in the balance of long-term demand and immediate demand, the optimization of procurement decision-making scheme, and the accuracy of patient demand prediction in a long period.

The panoramic perception of medical equipment operation and maintenance status is the basic guarantee for the implementation of smart medical care, the machine learning algorithm-based autonomous perception and active early warning model of medical equipment operation and maintenance status is proposed. Introduce deep learning multi-dimensional perception of medical equipment multi-source heterogeneous fault data training sample characteristics to realize autonomous perception of medical equipment operation and maintenance status, introduce reinforcement learning to realize autonomous decision-making of test sample fault characteristics, and build the active early warning mechanism for medical equipment faults. Taking the equipment department of hospital as the carrier of model effectiveness verification, the effectiveness simulation of the model was carried out, the results show that the model has the advantages of comprehensive fault information perception, strong compatibility of medical equipment, high efficiency of active early warning.

Strengthening the supervision over the use of large-scale medical equipment is an effective means to improve the efficiency of equipment use and the quality of medical services, and it is an important part of promoting the construction of the Healthy China and the development of health undertakings. Through four stages of preliminary demand investigation, intelligent collection of data, intelligent analysis and evaluation, and continuous improvement, a large-scale medical equipment intelligent management platform was built in our hospital. Real-time data collection, interconnection, analysis and evaluation were achieved, which could help the use and supervision, improve efficiency and effectiveness, and optimize the evaluation system.

Objective:To master the quality control status of high-risk medical equipment of medical institutions in Zhejiang province, and to put forward improvement measures to promote the quality control level of high-risk medical equipment.Methods:All the tertiary public hospitals, second-class public hospitals and private hospitals in Zhejiang were investigated. A questionnaire survey was conducted on the quantity, quality control methods and quality control specifications of high-risk medical equipment used by the medical institutions. Excel was used for data collation and descriptive statistical analysis of the questionnaire.Results:The quality control methods of high-risk medical equipment in 96 medical institutions in Zhejiang had different tendencies. The tertiary public hospitals tended to conduct quality control on their own, and the second-class public hospitals and private hospitals tended to choose third-party testing institutions for quality control. In addition, there were irregularities in quality control of some medical institutions. 16% of the quality control equipment in self-control mode was not traceable, and 2% of the third-party testing organizations had no relevant qualifications.Conclusions:Medical institutions in Zhejiang generally have a strong awareness of quality control of high-risk medical equipment. But there are some unsolved problems remained in quality control. In the future, we should continue to improve the quality control level of high-risk medical equipment based on medical measurement, as well as to focus on the improvement of the technical ability of medical equipment management departments, the traceability of quality inspection equipment, and the supervision and spot check of third-party testing institutions.
